Located on the Zuckerberg San Francisco General Hospital (ZSFG) campus, San Francisco Department of Public Health’s Family Health Center (FHC) is the largest primary care clinic in the San Francisco Health Network, with 12,000 patients through more than 58,000 patient visits per year.
The Family Health Center provides comprehensive health care services including health promotion, disease prevention, urgent care, continuity primary care services, and special access clinics in our commitment to holistic care of the entire family. The staff of dedicated medical professionals provide services that are accessible, culturally appropriate, safe and welcoming for a diverse community of patients including children, adolescents, adults, the elderly and homebound patients.
We provide multiple services in one location. It is the home for the Youth and Young Adult Clinic, a specialized clinic for those ages 12-25. FHC also houses the Newcomer Health Program providing health screenings and support services to newly arrived refugees and asylees living in San Francisco, San Mateo and Marin Counties. We also offer substance use counseling and treatment services through the FHC Bridge Clinic.
