We understand that older patients have unique needs. Our team works together to provide care that looks at the whole person, not just the medical issue.

We want our patients to feel supported during their visit and after they go home. That’s why we connect our patients and their families with follow-up care and community resources that help support their health needs. Our goal is to make sure every person under our care feels respected, well-cared-for, and confident about their health.

Services We Offer

  • Care designed for older adults to make the emergency department visit as comfortable as possible and avoid hospitalization whenever it is safe to do so
  • Comfort items and activities that don’t involve medicine or medical treatments, like stuffed animal puppies, fidget aprons, coloring books, puzzles, books, magazines, and cardboard sewing toys
  • Safe and personalized discharge plans for our patients when they leave the hospital.
  • Connections to community resources and local organizations that support health and well-being at home
  • Dedicated patient navigator and social worker who connect our older adults to their primary care providers and offer support
  • Specially trained volunteers, every day of the week
