Register for MyChart to stay connected to your health.
Sign Up Online
Go to MyChart Website at MyChart.sfdph.org
Choose one of these options:
- Sign up with an Activation Code.
- Type activation code
- Type your ZIP code and date of birth
- Follow the instructions to create your account.
- Sign up without a code.
- Verify your identity and set up your account by following the steps
- Sign up using a link sent via email or text:

- Create a username and password
- Enter your date of birth
- Click “Submit” to finish registration.

- Open the App:
- Agree to the Terms & Conditions
- Search for organization:
- Type “San Francisco Public Health”
- Click “Sign Up Now”
- Choose:
- Use an Activation Code
- Sign Up Without a Code
- Complete Set-up:
- Enter your information
- Create your username and password
- Follow prompts to complete setup.

Having Trouble Logging In?
Forgot your MyChart login details?
- Go to the MyChart login page
- Click on “Forgot login information”
- Select either “I forgot my username” or “forgot my password”
- Follow the prompts and check your email for a link to reset your login information
